Dental implantation

Dental implantation is an operation to restore lost teeth by implanting a titanium pin, an artificial tooth root, into the jawbone, and then installing a dental crown.

Reasons to do implantation

Lack of teeth leads to unpleasant consequences, even if it is visually imperceptible. To the place of a lost tooth, neighboring ones are shifted - the bite and aesthetics of a smile change, facial features are deformed with them. The remaining teeth get an uneven load, quickly erased and destroyed. As a result, the preservation of the teeth becomes a difficult and expensive task compared to the implantation of a lost tooth immediately.

Alternatives to implants - dentures and bridges, can perform aesthetic and chewing functions, but they often require turning of the adjacent teeth, and instead of one lost tooth, two more become threatened. Orthopedic structures need support - adjacent teeth, it is they who take on the additional chewing load and wear many times faster. In addition, any additional structures in the mouth complicate hygiene and contribute to the proliferation of bacteria, that is, increase the risk of caries and periodontal disease.

Implantation is a modern high-tech way to fully restore a tooth with its aesthetic and functional properties.

The implant made of titanium is fully compatible with the body and acts as the root of the tooth and takes over the entire chewing load. The crown is responsible for the aesthetics, which is matched to the color and structure of the patient's teeth enamel and looks as natural as possible.

Benefits of implantation

Durability

97% of implants take root and replace lost teeth for decades.

Health of adjacent teeth
When chewing, adjacent teeth are not subjected to additional stress, are not destroyed or shifted.

Naturalness and aesthetics
An artificial tooth is no different from other teeth. Installing the pin does not affect the appearance of the gums.

Convenience
The implant can withstand the chewing of solid food and is not felt as a foreign object in the mouth.

Timing of implantation

The operation of installing the pin takes 20-30 minutes, but preparation for implantation is a long complex event.

Classical two-stage implantation can take from 4 months to 1 year. Such a period is required for the engraftment of the implant in the bone tissue, since it is a completely foreign object for the organism. Only after full integration with the bone, the implant will be able to take on the chewing load with minimal risk of rejection.

Often, with a long absence of a tooth, the bone tissue at the site of the lost tooth decreases, and the patient requires bone grafting - the build-up of additional osteomaterials. Read more about preparing for implantation here.

In some cases, simultaneous implantation is possible immediately after tooth extraction, as well as with full edentulous implantation of All-on-4 with the installation of conditionally removable denture immediately.

The optimal method of treatment is selected by a surgeon after a complete diagnosis.

Implantation cost

The price of implantation depends on the type of implantation, selected implants and crowns. Cost may also be added to the price of a sinus lift or bone augmentation surgery.

Types of implantation

1. Classical implantation

Implantation takes place in 2 stages - surgical, when the pin itself is installed, and orthopedic - crown installation. The gap between the stages is 3-6 months. It is necessary for the full integration of the implant. With such an operation, the risk of inflammation and rejection is minimal, and at the end of the process, the patient can fully chew food and not worry about appearance.

Stages of classical implantation

Preparatory stage

The patient undergoes a consultation with a surgeon, takes an X-ray and a computerized tomogram, and tests to determine the state of health and identify contraindications.

According to the results of the diagnosis, the doctor draws up a treatment plan, assigns additional measures as necessary: sinus lifting, professional hygiene or sanitation.

Implant installation
on the results of all preparatory procedures
    1. Anesthesia is administered to the patient - its action is sufficient for complete painlessness of the procedure.
    2. An incision is made in the gum for access to the bone
    3. A special mill mills a hole into which the implant is screwed.
    4. After installing the pin, the gum is sutured.
    5. The stitches are removed after 5-7 days at the control examination.

The implant adapts root within 3-6 months.

Installing the gum former
In 3-6 months
To make the gum look naturally with a new crown, a gum former is placed on the implant.

Installing the abutment and crown
2 weeks after installing the gum former
The abutment replaces the gum former after 2 weeks. The individual crown is made from the patient's casts and is carefully selected. After all the necessary fittings and corrections, the final version is installed on the abutment.

2. Immediate implantation

Implantation in one visit with simultaneous installation of the implant and the crown is carried out immediately after the removal of the tooth. The pin is placed directly in place of the root of the extracted tooth without an additional incision of the gums and bone. A temporary crown is attached to it, and after 3 months it is replaced by a permanent one. This is necessary in order not to load the implant right away and to allow it to settle down.

Simultaneous implantation is convenient and saves time, but certain conditions are necessary for its realization: sufficient volume and good bone density, a healthy periodontal and neighboring teeth - they will take over the entire chewing load in the first months.

Untreated tooth extraction

Installing the implant, abutment and temporary crown

Installing a permanent crown in 3-4 months

Does it hurt to do implantation?

Dental implantation is a simple and very common operation in modern dentistry. To exclude any sensitivity during surgery, local anesthesia is sufficient.

After surgery, the patient can normally go about his business. To relieve pain in the first days after surgery, you can use painkillers, which the doctor will recommend.

Contraindications to implantation

Absolute contraindications for which the operation is impossible:

  • Malignant tumors;
  • Tuberculosis;
  • Severe diseases of the cardiovascular, nervous and endocrine systems;
  • Impaired tissue repair, for example, due to immunodeficiency or after chemotherapy and transplantation;
  • Mental illness, alcohol and drug addiction.

The relative contraindications for which the operation should be postponed until the problem is resolved:

  • Diseases of the oral cavity: caries, periodontitis, tartar, cyst;
  • Poor hygiene (increases the risk of rejection)
  • Active smoking (increases the risk of rejection)
  • Incorrect bite (impossible to place the implant correctly, not enough space for the crown)

Features of rehabilitation after implantation

  1. After the operation, it is necessary to exclude too hot, cold, spicy and hard food. Try not to chew the side on which the implant is placed, do not use toothpicks;
  2. It is recommended to stop smoking for a month so that the tissues can recover;
  3. Limit heavy physical exertion;
  4. Avoid temperature extremes: severe hypothermia, visits to the sauna.
  5. Brush your teeth gently without pressure on the site of the operation, use a rinse and irrigator.

It is normal to experience discomfort and pain in the first week after the operation - the body recovers from injury. Temperature, slight bleeding and redness of the gums may rise.

Edema and bruise disappear in 5-10 days.

Complications after implantation

Implantation is a safe procedure and in most cases takes place without serious complications. After surgery, edema, temperature or bleeding may occur - this is a normal reaction of the body and does not require a visit to a doctor.

Serious complications are:

  1. Implant rejection. It is characterized by a feeling of mobility of the implant in the jaw, persistent pain. Rejection usually occurs due to lack of bone material, poor hygiene, infection or smoking. In the rarest cases, the cause is an allergy to titanium.

    Modern implants are created using the latest technologies, are tested and take root in 97% of cases. The risk of rejection, as a rule, increases due to non-compliance with the rules of care.

  2. Periimplantitis is inflammation around the implant caused by an infection. It can be identified by severe redness, growing edema and suppuration at the site of surgery. Infections are recorded due to poor diagnosis, for example, unsung caries on the adjacent tooth, or poor hygiene.

Modern implantation is very technological and virtually eliminates any complications - periimplantitis or rejection occurs in 1-3% of cases. Most often, even after complications, you can install an implant, if you neutralize inflammations in time, take additional measures to improve the conditions for implantation, and observe careful care.

Implantation is a reliable way to restore teeth for a long time without compromise and inconvenience. Implant manufacturers care about the quality of materials, constantly improving technology. The task of the patient is to find a doctor whom he will trust and follow the recommendations. In this case, the operation and adaptation to the implant will be calm and painless.
